Referring Vets

 – Use when referring patients for in-office care – 

Veterinary referrals are preferred for all consultations, but not required. Please complete this form if you refer a patient for any type of in-practice care.

– Use when consulting directly with our office –

Veterinarians wishing to consult directly with our practice regarding their patient or their own pet may schedule either fifteen- or thirty-minute phone or Zoom consultations. If you wish to consult about a mutual patient, please contact our office directly at 732-268-8553.

Veterinarians are billed directly for these services

– Use when referring patients for virtual care –

Veterinary referrals are required for virtual appointments. When a distant client requests a Tele-Advice session, we require a referral from an RDVM that holds a Veterinarian-Patient-Client-Relationship (VPCR). As we are consulting virtually and do not hold a VPCR, we are unable to treat or diagnose disease, or make treatment or supplement recommendations.

– Use when consulting directly with our office – 

Veterinarians wishing to start their patient on Intravenous Vitamin C Treatments or Mistletoe Treatments may request an individually tailored protocol for their patient via this form. If you wish to consult with our doctors before making that decision, please submit a Veterinarian Tele-Consult Request first.
